Healthcare CEOs are hearing plenty of promises about AI scribes. Vendors promise executives that their products are “integrated,” “seamless” and “time-saving.” On paper, it seems all good. However, in reality, the difference between a solution that simply hooks into your EHR and one that truly transforms clinical workflows is what I call the last mile of AI scribing.
Why The Last Mile Matters
The final mile is the live test of the technology. It is the difference between a technically recorded note and the accuracy, compliance and trustworthiness of that note by the clinician who has to sign it. Many solutions do not travel even that far. And when they fail, it’s not just technology that fails, but your physicians, your revenue cycle and eventually your patients.
In a recent study of AI transcription tools, word error rates (an indication of accuracy) are reported to range from as low as about 8.7% in controlled dictation conditions to over 50% in more conversational or multi-speaker scenarios, demonstrating the great variation in accuracy due to the environment and workflow. That is a good sign, but the technology still needs major development. Specifically, they need to be trained in the environment and with the people they will help, not a general dataset.
Integration is a necessary step, but not sufficient. Just because a note is in the chart does not mean that the technology has solved the problem.
If doctors are still working after hours fixing errors, claims are still getting denied or downcoded, and patients are still watching their doctor look at a screen and not them, then the investment has not been well spent. Integration must eliminate the connectivity issue. And the last mile answers the adoption question.
Where Solutions Fail After Integration
Even well-incorporated AI scribes can make mistakes in the critical last mile. Some of the most common slipups include:
• Garbage in, garbage out. If the AI gets a clinical nuance wrong or unclear, then integration only helps spread the error more widely and rapidly across the chart.
• Workflow mismatch. When a scribe forces you to click multiple times, switch between pages or conduct heavy edits after the visit, it is not efficient, but only creates more friction. Some interviews with physicians have shown that the common complaints for qualitative use of AI scribe tools are editing and note style, such as length and structure.
• Compliance risks. Using incorrectly populated structured fields may look good in the chart interface, but can lead to denials, audits and financial information breaches.
• Trust gaps. If clinicians do not trust that the note will accurately capture their nuances or accurately code what they are seeing, then they will revert to double-documenting. That defeats the entire purpose.
If these issues are not addressed at the last phase of integration, then even the best AI would fumble.
What Success Looks Like In The Last Mile
So what should success metrics resemble once the integration is finished?
1. Accuracy that improves with use. AI models should adapt to your clinicians, specialties and documentation standards. If the accuracy of the approach starts to level off, so will the adoption.
2. Minimal corrections required. Notes must be completed at least 90% to 95% in the first pass; it should not require continual editing. In fact, modern AI scribes now achieve approximately 98% accuracy for general medical terminology and about 95% for specialty terms. However, many of those drafts need to be reviewed by the clinicians.
3. Revenue cycle resilience. Documentation should be able to withstand auto-coding and payer audits.
4. Physician adoption and satisfaction. Doctor acceptance and doctor satisfaction. Burnout rate must be getting better, not worse. The applications of the tool are only as good as how satisfied and relieved the doctors feel. Overall, we have seen promising results so far. According to one study, 84% of physicians reported AI scribes had a positive impact on patient interactions, and 82% of physicians reported a positive impact on overall work satisfaction.
5. Better patient experience. Better patient experience. When physicians are no longer spending the time writing, they can spend more time looking patients in the eye. That’s what truly matters.
The Executive Imperative: Proof Beyond Integration
Healthcare leaders must push the vendor to go beyond the “integration demo.” You need real proof that it is effective in the reality of everyday clinical practice.
This proof can come in many forms, such as time saved per encounter (measured over months) and physician satisfaction ratings (before and after deployment). It can also be found in audit and denial data showing impact on the revenue cycle and specialty-specific accuracy benchmarks, not just generic averages.
If vendors can’t show this proof, they haven’t solved the last mile.

That was never the end goal of integration. It was only the starting line. The last mile of AI scribing must be where technology lives up to its promise, and doesn’t quietly become just another headcount cost center with a disguised UI/UX.
Healthcare leaders need to hold themselves and their AI scribes’ vendor accountable. Every provider who incorporates an AI scribe into their care model must have a method to prove that their solution works and produces the promised time, trust and financial performance gains. Because if the last mile can’t be solved, all else is irrelevant.
And in healthcare, where we have to make every hour and every dollar count, half a solution isn’t even a solution.
